How does the Bouquet operate differently from a two bladed ‘duckbill’ speculum?
This device has two parts and five petals instead of two parts with an upper and lower blade. It is held like this:
– Once the speculum is inserted in a closed position (see figure below) to resistance, the dilator is pushed forward with the thumb and engages with the locking
mechanism. You may hear a slight click as the dilator moves forward into 5 progressively wider dilations of the petals.
– Always insert the speculum with retractor all the way BACK and the tip rounded and closed like this:
– Once finished with the procedure or screening, to remove the speculum, push down on the release tab at the top of the dilator and retract it back. Be sure to keep
one hand on the handle as you are retracking the dilator. With the dilator fully retracted, you may now remove the entire speculum.
How should I learn to use the Bouquet?
Before using the Bouquet Speculum for the first time, we suggest reading this guide several times and opening and closing a Bouquet Speculum to observe the five stops and how Part B, the dilator, opens and closes the 5 petals in Part A, the Speculum.
• There is no need to push down on the release tab when advancing the dilator. The locking mechanism at the top of Part B will engage each of the 5 holes successively with little force as you advance the dilator.
• To retract the dilator, hold the handle with one hand and push down on the locking mechanism (Part B) with your index finger of the other hand. Simply grasp the dilator with the tab pushed down and back it out slowly. The pressure from inside the vagina will help to close the petals and “squeeze” the dilator out. The entire speculum can then be removed from the vagina.
What if I have trouble with the initial insertion?
Be sure that you are inserting the speculum closed. The rounded point should be closed up and 1 cm wide and it should be liberally covered with lubricant.
If the provider is having difficulty inserting the tip of the Bouquet Speculum, we suggest a bimanual (digital) exam first, thereby expanding the introitus and gaining a sense of the position of the cervix. The vagina has a depth of, on average, 2.5-3.5 inches. This represents an insertion of the Bouquet Speculum at only half of its the full length of the petals of the speculum for most women.
Low BMI, Smaller Patient:
For smaller woman, adolescents, and children, the Bouquet Speculum closed tip need only be inserted an inch or two until resistance is met and then the dilator can be engaged at the first or second stop. The speculum is slowly opened, stop by stop by pushing forward the entire dilator with the thumb until the cervix is visualized.
High BMI, Larger Patient:
Larger and multiparous women may have deeper vaginal vaults. A full insertion of the Bouquet speculum allows for a vaginal depth of 4.5 inches. Fully inserted and then opened the speculum looks like this (see dilator is pushed all the way forward in the images below). At this point, if greater visualization of the cervix is needed, advance the entire speculum an inch forward to have the tips of the petals cup and envelop the cervix which is typically about an inch in depth.
What happens if provider inserts the speculum to the point of resistance and tries to open by pushing retractor forward resulting in inability to open?
The provider should back the entire unopened speculum about an inch back and gently push the dilator forward until the cervix is visualized.
Once the cervix is visualized and the dilator is maximally dilated (last stop), the entire Bouquet speculum should be advanced about one inch so that the tips of the
5 “petals” encompass the entire cervix, thus preventing any vaginal tissue collapse.
What if the provider has trouble visualizing the cervix?
The beauty of the Bouquet Speculum is that, with the 5 petals and radial opening, the cervix always comes into view. One of the limitations and barriers to the 2-
bladed speculum is its opening in a vertical (anterior to posterior) direction. When a cervix that is retroverted or significantly anteflexed or off to one side or has
redundant vaginal tissue makes it a difficult procedure to locate the cervix without repositioning the patient.
